If anyone is interested, they can PM me with their email address, and I can send you some Word docs for release forms (sorry, I have no hosted site as of yet).

I've used this for a couple of projects I had to do (street bike video and paintball tournament).

So I just thought I'd contribute for those in need.

-Location Agreement
-Music Release Form
-Talent Release Form
-Waiver Risk Indemnity
-Audition Information

EDIT: Just to be clear, this does not absolve anyone from being sued or to excuse you as a producer or company from obtaining insurance if need be. These are more "good faith" agreements for you to use as appropriate.

It's basically a form that anyone invovled in your production to hold you and your company from liablity to your filming of whatever, and that they assume a level of risk on their part knowingly.

iirc, it basically says that you're not responsible if something goes wrong during filming, and people participate at their own risk.

(Edit: yeah, what he said wink )

Have a look at this thread for a place to upload too. Or look at the Ultimate FAQ ( sticked at the top of the page ) for some more useful links.. biggrin

yeah, but what i hate about those sites is everybody knows where you are and can access your stuff, even predators and other creeps on the internet. while it's not a whole lot better, i use geocities and google pages for my hosting.

Just a note to users in the UK.

We found out when doing commercial filming that just having someone waiver is no longer good enough in a court of law - it stands for very little. You still require full insurance as if they had not signed a waiver - which is very expensive.

rofl. You make it sound as if the UK's a really bad place to live because of that.

I think the law's pretty fair. If someone's doing something dangerous on behalf of a film then it makes sense for the filmmakers to bear some responsibility.

I'm not being harsh about this when I say this (just making a point, and you may have been joking) but on that side of the world, they make some great TV programming that the U.S. has adopted. The Office, and American Idol to name a few...

okay, maybe it doesn't suck there, but it's stupid that you have to get full insurance.

http://sonnyboo.com has some forms that are very useful if anyone is still looking. PS- Great Britain is awesome, but I don't agree with that policy.
